In this way this form is ready in Microsoft Word. Now store this patient form in your computer by following the steps given below.
Step 1: Save the above Patient Form by some suitable name. Here is an illustrative screenshot. Click on File Option then a dailog box will appear that will allow you to save your Word document. Then click con Save as option to save your “Patient Form” in Computer.

Step 2: Another dialogue box will appear that will allow you to enter your form name to save the patient form by this particular name, then click on the Save button to save your file.
